APPOINTMENT TIMES

Monday - Thursday 9am - 5pm
I do not schedule client appointments on Friday.
If you are going to be late, please call to let me know you are on your way. If you are 15 minutes or more late, or need to leave 15 minutes or more early, the session will be cancelled.
If you need to cancel, please inform me at least 24 hours in advance to avoid a being charged for the session. You reserve that block of time and often people are waiting for openings to come in.

| FORMS & POLICIES |
Save time by filling out the following 4 forms before your appointment.
Option 1 (preferred) . . . .allows you to fill in the forms on your computer and either send them to me BEFORE your appointment by attaching them to an e-mail. They are in Microsoft Word. If you do not want to send them by e-mail, you can sill fill them out, print them and bring them with you. (They have shaded boxes on the form that expand as you type.)
Option 2 . . . .allows you to print out the forms and fill them out by hand. Bring them with you to your first appointment.
If you have any questions, let me know by e-mail so you can receive your answer before you sign any forms. We can also discuss any questions in our first meeting:

4. Notice of Privacy Policies (this one is for you to keep and is a PDF file)
5. Informed Consent for Counseling (this is still a PDF file that you need to sign and initial by hand. You do not need to bring it to the first session, but DO need to bring it if you decide to return and continue working with me.)
Option 2: (forms to print out and fill out by hand)

Informed Consent for Counseling (This form is for both of you to review, initial where indicated and sign. After our first session, I send people home to decide if they want to continue working with me before we make a second appointment. If you prefer to wait and sign a copy of this form in the office and receive a copy for yourselves, you can do that, but please take time to review this ahead of time.) There is a correction about charges for appointments not cancelled 24 hours in advance. I am now billing the full amount of the session because other people who are waiting are prevented from getting in and because I want people to make this a priority.
CELL PHONES:
I expect you to turn off phones before we start each sessionduring your session. I do understand that sometimes there are sick children or family medical concerns and we can discuss that. Please make arrangements so that you will not be expected to take work calls. The intrusion of constant availability is part of the difficulties in many relationships!
CHILDREN: Please make arrangements for someone to take care of your children during your appointment. Children are not permitted in session or to be left in the waiting room . I have tried it in the past, but it is a constant pull on attention and invariably ends up interrupting the session. Learning to be more fully present to your partner is one of the skills most people need to build and to create time to do.

| HOME STUDY PREMARITAL COURSE |
Click here for the Premarital Course website. (This course meets the requirements for Florida's marriage license law in numerous counties. Check to see if your County is on the list.) However, some couples do not need it for requirements, but want to do the course for their own benefit. It has good information for any couple, activities to do together, and discussion questions about support, friends, family, children, sex, finances, and more.
